Fentanyl is a pain medication that is 100 times more powerful than morphine. Since 1990, Johnson & Johnson has used Fentanyl in its Duragesic® brand pain patch.  

In 2004, five lots of Duragesic® were recalled after a defect allowed the Fentanyl gel from the patch to leak out.  As a result of the leak, Fentanyl could be absorbed into the body at dangerously high, even deadly, levels. The FDA received hundreds of fatality reports stemming from the use of a defective patch.

Despite the recall, defective Duragesic® patches continued to be manufactured and distributed. In February 2008, Johnson & Johnson had to recall another approximately 32 million patches due to defects that can result in a leak. Due to the nature of the leak, caregivers administering the patch can also be exposed to the deadly levels of Fentanyl.
